By filling this out and signing below, you are retaining Integrity Roofing Services, Ltd. to document and process your insurance claim, and authorizing Integrity Roofing to communicate directly with your insurance company about it. This does not commit you to any dollar amount or cost -- it simply lets us get started on your claim. As required by Colorado law, we cannot pay, waive, rebate, or credit any part of your insurance deductible.

Part A — General Commitment

I/We hereby retain Integrity Roofing Services, LTD for assisting in the documentation and processing of my insurance claim so that my insurance carrier can assess the fair value of my loss and work directly with my roofer. My insurance company is authorized and directed to discuss my claim with my agent. I/We have hired Integrity Roofing Services, LTD to assist me in making this claim so that I can avoid the inconvenience and paperwork associated with the claims process. I direct that my insurance company deal directly with my agent in arriving at the value of my loss. I direct my insurance company to include Integrity Roofing Services, LTD on any and all scope of loss documents pertaining to my claim. I/We authorize Integrity Roofing Services, LTD to proceed with all repairs in accordance with all scopes and parameters authorized by the insurance company, as per insurance adjustment, as soon as the insurance company approves the claim. I/We agree that Integrity Roofing Services, LTD may complete such additional repairs or use additional or better materials than authorized by our insurance and we agree to pay this additional amount as reflected in our written approval of upgrades to the material or labor, if any. Pursuant to 6-22-101 Colorado law requires we inform you: You the property owner may rescind this contract and obtain a full refund of any deposit within 72 hours after entering this contract. And since property owner plans to use the proceeds of a property and casualty insurance policy issued pursuant to part 1 of article 4 of title 10, C.R.S., to pay for the roofing work, pursuant to section 6-22-105, the roofing contractor cannot pay, waive, rebate, or promise to pay, waive or rebate all or part of any insurance deductible applicable to the insurance claim for payment for roofing work on the covered residential property. Integrity Roofing Services, Ltd and its Authorized Representatives are not Public Insurance Adjusters. CONTRACT IS NULL AND VOID IF THE INSURANCE COMPANY DENIES COVERAGE.
